Dear Sir/Madam,
I am writing to express my concern regarding multiple claims that have been consistently rejected by Discovery Medical Aid, despite having submitted all the required documentation on several occasions.
Attached, please find the proof of payments and claims for various medical expenses related to my condition. These include invoices from Clicks, Coloplast, Dischem, Ayoba Pharmacy, and Medirite, as well as documentation for a urodynamic study conducted by Dr. ***ema — which was specifically requested by the medical aid for confirmation of diagnosis.
Despite the chronic/PMB condition being backdated and approved from 24 March 2025, I have not been reimbursed for 12 months’ worth of out-of-pocket expenses related to:
Catheters
Micrabegron/Urton
Diagnostic procedures
All claims have been submitted through the appropriate channels and are included again in the attachments for your further investigation.
As a very concerned patient, I kindly request a thorough review and urgent feedback on why these claims have been rejected and what immediate steps will be taken to resolve this matter.

I am writing to follow up on a reimbur*****t for medical expenses. I previously submitted invoices and proof of payment for Urton (where Bertmiga was dispensed twice, subject to availability), including those from Coloplast, but I have not yet received any payment.
For your convenience, I have re-attached all relevant documents, including those for catheter purchases. These expenses were incurred at Clicks, Medirite, Ayoba Pharmacy, Dischem, and directly from Coloplast. Despite these valid claims being submitted after the patient visited these providers, they were rejected by the medical aid, and I had to pay out of pocket.
These outstanding claims date back to March 2024. As requested by Nozuko from your team (***), I have also attached clear copies of the invoices from the relevant medical and pharmaceutical suppliers.
Given the significant delay and inconvenience caused by these backdated invoices, I have copied the relevant parties and authorities below who may be able to assist.
