Pension - Medical aid cancelled

Good day,<br> <br> I am at a loss and need a competent person to assist me.<br> <br> I worked for the Tshwane Municipality and went on pension on the 30.4.15, after phoning and completing the \Transfer from active to retiree status"forms (stamped and signed by the Municipality) and sending these forms through in March and confirming that same was received by the medical aid"
