ONE STAR RATING FOR DISCOVERY HEALTH AGAIN, INCORRECT FORM

<p>i went for my bi-annual check up on the 26 May could be the 25 May 2017. I have no Thyroid gland and I lost the war with my Eltroxin being paid on chronic. I had Asthma as a child and I thought it was manageable so I never gave it much thought. However, my airwaves are being restricted and can result in all sorts of problem. I need preventative medication to solve this problem. Dr fills in a Chronic Medication form and it was sent through twice on calling Discovery today, guess what. The form of 2015 was used so nothing will be done. Yes, nothing will be done. No communication with the Dr or me as the patient. I made this miraculous discovery this morning. In other words when the wrong form is used you as the Patient can die as they will not contact the Sender to inform them of the situation. True Statement: Discovery Makes me SICK</p>
