How can this happen?

Received a Whatsapp message from Discovery thanking me for selecting Dr AJ VAN WYK as my primary care doctor. Something amiss... I saw this dr once, about a year ago. Never selected him/probably never will as my primary care doctor.

Can you please clarify how/why this happened. Am concerned that something like this can happen without any action/knowledge from myself.
