Discovery refuses to pay for opeation

My wife went to hospital, had all the relevant pre authorisation and pre assessments.

Discovery even helped the provider with the invoice, yet they refuse to pay the hospital bill. What is the point of medical aid if they refuse to pay for hospital visits?
