Discovery Pre-authorization - what does it actually mean??

Very disappointed with Discovery as I had obtained approval regarding my pre-authorization prior to my procedure, then once the procedure was done, i get a statement from the hospital stating that Discovery had declined the claim regarding the theatre fees, I have queried with Discovery and was told it was being investigated,, then received an email stating that my authorization is declined without giving me clear reasons as to why it was declined.
