Discovery health knowing full well members condition yet discovery allows incorrect Z ICD 10 code used imstead of correct N311

I hope this message finds you well.
I am writing to formally appeal the rejection of payment for the attached urine test, which I believe was due to incorrect ICD-10 codes being app****. As discussed in a call I received last week, this seems to have been an administrative error.
The urine test in question was medically necessary and aligns with the treatment requirements outlined in the decision letter I received from Discovery. This test was related to a bladder infection, which is a known complication of my medical condition—neuropathic bladder—as supported by the attached medical documentation. The infection is a direct result of intermittent catheterisation, which I am required to perform regularly.
Additionally, I have requested a supporting letter outlining my medical condition, which will assist in gaining access to disabled and hygiene-specific facilities due to the ongoing nature of my condition.
I kindly request that you reassess this claim with the correct ICD-10 coding and process the payment accordingly. Please confirm receipt of this appeal and advise on any further information you may require.
