Discovery Declines Procedure and expects a Dr to be ********* to change codes

My daughter are struggling with severe nose bleeds. Authorisation was obtained for her to see the ENT on which he confirmed the artery must be fixed in theatre because of the severity of the case. Before visiting the ENT bloodtests were done and the are low iron levels due to the loss of blood. The video of the nose bleeds were sent to Discovery with the Motivational letter. Due to 1 word the Dr used Discovery closed the case and are awaiting New Codes. The ICD-10 code are a PMB Code and 2 of the 4 procedure codes have been approved. Discovery expects the Dr to be ********* and change codes for 2 of the procedures. When burning / closing an artery with a Rhinoplasty as procedure for nose bleeds it is evident that no plastic surgery, reconstructive surgery or that it is a cosmetic procedure. Another code they mention shows functional reconstruction. How can an ENT perform these procedures if this is the work of a Plastic Surgeon.

Will Discovery only react when my daughter bleeds to death and get admitted for days?
