Claims not paid

I was taken to hospital on the 22nd August was admitted for 1 night at the life Glenwood hospital. I have been in contact with discovery since I was out of hospital to make sure the Bills get paid and every time I am told something different. First it was the pma form then eventually after the doctor decided to actually complete the forms discovery received them in November. I called to follow up and I was told it will be sent for authorisation and I should have feed back in 2-3 working days. I called and still nothing was done then they wanted the claim from the hospital, I called the hospital and the lady said it was sent however she will send it to me to send to discovery maybe this will go faster if I send it. I sent it to discovery and called to follow up and I was told it wasn't received so I sent it again only this time to claims adjustments,followed up again and was told only half of the document was received.? It's one pdf document of 4 pages with the first being the icd10 codes! Called again discovery called the hospital to put the codes on every line? Then I was told reason they rejected was due to the discharge date?
