Claim not paid despite procedure approved

My wife was admitted to hospital with authorization. But when claims were submitted, discovery refused to pay (they claim the wrong codes were used), but they authorized it... We have queried this 3 times in the last 3 months, without any luck/success/acknowledgement (call it what you want)...
