Claim denied

My claim has been denied for two reasons, one that is was submitted too late and the other for wrong code. I have never had issues regarding a claim for this Doctor. This is unacceptable as I am paying nearly R3000 a month for my medical aid and that the money in my medcial account is actually my money and no I get denied and with my chronic there is a R5 odd co-payment on an already generic medicine. This is actually theft.
