Case Reports Not Supp****

I have been trying unsuccessfully to get case report forms from Discovery Health. After several telephone calls, there is still no response. This has been on going since 08Jul2015 - there is a time limit on the receipt of these forms which need to be submitted for an insurance claim.

I also noted that our service consultants have been sending these from our system directly and confirmed that we did not have your correct email address, hence the non-delivery of your request.
