Can’t provide basic information!!

I was sent an application to fill in by discovery for my new born. On the 27 March n I sent it back with all the required information on the 27 March. I just called in today just to check if my baby’s application is correct only to find out that i had to submit to HR to fill in. Why was I not notified if there were outstanding information???

I spoke to someone at discovery and that person couldn’t even give me proper information it turns out I was sent a wrong form! Since they saw their mistake why am I not sent the correct forms?!

Can I kindly request that I get the correct forms and someone to explain the process?
