Cancellation of an upto date membership

On 31/7/15 I resigned from my place of employment. Upon assuming duties with the new employer I learnt that they do not have discovery medical aid as part of their schemes. wishing to continue with DMA I opted to register as private member.Knowing that I have a window of 03 months after the last premium before cancellation of my medical aid membership,I then went to the Centurion walkincenter sometime in Sept to have my membership continued under a private capacity. They advised my employer will engage directly with them for such to be effected. My new employer came back with a reinstatement form and indicated that I have to pay a two months' outstanding premium for the medical aid cover to continue. I made the payment and filled the forms I was given. only to learn in October that my medical aid appears to be suspended when I visited service providers. since Nov 02 I have had to pay a 3rd premium and filled 07 declaration forms but my membership has not been reinstated. no one bothers to provide any feedback and I have since had to pay a cash amount of R25 000 in cash 4 a medical procedure which I urgently had to undergo. To this date I'm sent from pillar to post without a cover

Thank you for your patience while we investigated this matter for you.

On 15 September 2015 we asked to terminate your membership due to a resignation from the previous employer effective 30 June 2015. The policy went into a conservation state and upon your visit to our Service Centre, the options to reinstate either via your new employer or own capacity was explained. On 14 October 2015, we received an email for disclosure of your personal information from a third party, we did not disclose any information due to confidentiality reasons however we did email you the amount due and health declaration(Due because of break in membership for more than 30 days) form.

We received your payments however the health declaration form was never received on any of the emails sent to us. Our Client Services Department explained on a few occasions that we require the form to move forward. We also tried calling you a few times to rectify this situation and unfortunately you were not available for us to assist further. A premium refund was issued as we were no longer able to reinstate your policy due to the break in membership and change in health status.
