Bad service

On the 18th of dec i send in my app for medical aid. 4 days later you phoned me regarding section not filled in correctly. I did give them the info over the phone and didnt hear from you till yesterday. Now there is a page i didnt sign. Why wasnt i inform on the this on the first time. <br> <br> My cover is sepose to start on the 1of jan but at this rate i doubt <br> Can someone please help!!!
