Approved PMB, but Dr consultation rejected

Hi, I'm writing about a concern with an approved PMB claim. My mom's medical aid is rejecting a doctor's consultation visit, saying funds are depleted. However, she was admitted and discharged from hospital in Jan, before the PMB was app**** for (approved from Feb). Why is the past admission being linked to the PMB, when it's outside the approved date range?
