Doctor and nurse lies on referrals letter

After 4 hours of waiting outside in the cold with my 6 year old Daughter later diagnosed with appendicitis, the doctor told me they couldn't assist me and she **** on my application form saying I have transport to the hospital they were referring us to so that they did not have to arrange an ambulance for us.
