U19 taking forever to be sent to me by DirectAxis

Direct Axis really has failed me as the former employee. Since July they still haven't sent my U19 form. On the 6th of this month they rep**** that they will get back to me in 24-48hrs, nothing yet. I don't even know what to do now and I can't go to their office I'm heavily pregnant 😩
