Payment not showing on statement. Loan paid off

After many months of payments made and proof of payments there is still a payment not showing on the statement. Loan is paid off but no further assistance. How many proof of payments do i have to send through in order to prove the payment was made. I am very unsatisfied and do not recommend DirectAxis
