UIF19 & PAYSLIPS

I worked for Workforce Staffing in Montague Gardens; due to dissatisfaction, I decided to quit the job. I called them to give me a UI19 form and 3 months payslips so that I do not struggle when I need to claim my UIF. To date, I haven't received the letter. Even the department of labour is not assisting.
