Uif maternity leave

I submitted my forms for my maternity leave benefits on the 27th August 2020, I went to work on the 16 November. They haven't processed my forms till this day, when I went to complain they said I'd get it. I then app**** using ufiling but they responded requesting my forms, I don't have the forms as I didn't make copies. I lodged a complaint but they not responding
