still not paid out

I have app**** for uif in april and went back to check already for 3months and still haven't got paid,this month i asked what is the problem and they told me all the company has paid out but waiting for labour to approve,i went there only now they tell me that they didnt receive ui19 form for 2010 from Kelly,when i call kelly they tell me they dont have ui19 for 2010 for me,so i would like to know if i wont be getting my uif money because of ui19 that doesnt appear,i will like somebody to help me please to get uif cos i am currently unemployed and depend on those funds until i get work,,,,please please somebody help me please
