Not receiving money and get send back to doctors every time

The uif send me back from Nigel to Pretoria every time as forms are not completely filled in. And at the bottom thevdoctor says I am permanently incompasitated. And they want monthly reports from the doctor but i am boarded permanently and have no funds to travel every time to Pretoria from Heidelberg and nobody wants to help me i received 2 payments last payment received was in November and they promise to get back to me but nothing happens no results.

I have no money to go to Pretoria everytime and my doctor said he must write on the form that i am permanently incompasitated. And the uif has all the documentation of my sickness. When i was supposed to receive my money in Dec they said they were offlinevand i never received it and in Jan they sent me to doctor 4 times and still they say the form is incorrect although the form says i am permanently incompasitated. The docor even stamped and signed all the forms but only the date is permanently because the doctor said he cant give me a date every time they want a report from him every month. Please help me to sort this out.
