non payment of sick benefit

<p>on the 22 of December 2016 my disability claim form for payment was delivered to the Chatsworth branch and to date my sick benefits have not been paid out to me. i have personally gone to the branch twice since then to query payment and was told that the forms were sent to the new branch that opened and staf have been on leave and the payments will be effected. i have also tried calling their offices however the phone rings continously and is never answered. please investigate and pay out my sick benefit as i have now waited over 3 weeks for the funds.</p>
