Non payment of 2 months of Maternity UIF

I app**** for maternity UIF at the labour department in Sandton, I was paid fo rthe first 2 months, when I submitted my continuation form it was lost by Gloria Mahlaule, I escalated this to Percy T****shi (manager) to date I still have not recieved any payment nor does anyone respond to emails or return my phone calls or pickup the phone.
