Good Day all.<br> <br> My name is Patience Mhlunzi. I have submitted my claim on the 20th November 2015. <br> On the 11/12/2015 I received an sms as follows: <br> <br> Dear Patience Khonelwa. UIF Medical Certificate and leave income outstanding contact labour centre case no:***<br> <br> And yet all documents were submitted.<br> Before workers can claim, they must get the following documents ready:<br> o 13-digit bar-coded ID or passport; <br> o form UI-2.8 for banking details; <br> o form UI-2.7; <br> o form UI-2.3 (application form) <br> o medical certificate from a doctor or birth certificate of the baby; and <br> o form UI-4 (follow-up form). <br> <br> <br> Again on the 25th December 2015 I received an sms stating my claim has been cancelled due to outstanding documents, again I went to your offices in Orlando west and was told all is in order and I should ignore the sms.<br> <br> I have been calling all the labour centre's and they never answer their phones, if they decide to answer I'm always told the same thing that my claim hasn't been approved till today.<br> <br> <br> I would like someone to contact me with regards to my claim and I need my claim processed today still.
