maternity leave

Hi my name is thabiso malewa id *** case no:*** I went on maternity leave 1 July 2015 and submitted my forms in august.then I went on the 16 September to submit my payment form but I didn't receive anything when I went to labour they told me my ui19 is wrong then I asked my employer to email the correct one which she did.every week I went to check but nothing is happening.the last time I went ignitius told me he is going to create my application.and I'm going to work next week the 1st of November.im so frustrated cusz I really spend a lot going up n down....now I'm going back to work I don't even have money for transport.cusz I was home for 4months. <br> <br>
