Maternity benefits claim reject with no reason.

I visited the Balfour labour department office to submit all forms request for my maternity benefits. Every time when I went back to check for an update I was sent back to go fill in a certain form cause it has been misplaced or had went missing by one official to the next. With my understanding the department sends an employee from the standerton office to Balfour since there are no offices. The process of actually getting all my forms took more than a two months. After sevaral phones calls to the department of labour standerton office (were I was given attitude) , I got a reponse that my maternity benefits application had been rejected. I still don't know the reason why my claim was rejected and iv never received money for my claim. I really don't understand what really happen with my forms.
