HELP

I app**** for my uif benefit last year June 2015. Till today no one is able to help me. They say my company did not send the salary schedule form. Went back to my company on a number of occasions and till today they say they sent it, I called the UIF call center and say they say never received it. I asked the call center to send me a sample of what the document looks like so I can take it to them but no help. Please, please can someone help.
