Delay in UIF claim

My name is Thobeka Sibisi.ID number *** and I app**** for UIF on the 18 July till today haven't received any claim and the department keeps telling m of missing documents and worse I app**** in Kempton but moved to Centurion recently and its very hard to get to Kempton park. The last time I was there they sent me back and said I must come back the following day but I told I only had this day cause someone offered to take care of my baby.

Recently advised UIF form for my previous employer is missing but I sent it through and the gave me back the original. I am due to start work on the 6th November and in so much lack financial cause I am on unpaid.Please assist me.Case no:***
