Wrong information on my i.d

My name is Thembisile Lindiwe Zwane, the below mentioned Identity number. I went to open a bank account and they told me I am married and I appear as Thembisile Lindiwe Moussa Zwane. I went to home affairs on the 13 April 2013 and was assisted and told that I should go to police station and do avidavit of which I did and they I had to pay an amount of R140 at home affairs to apply for a new identity document. I went to home affairs to check the progress for my I.d and they gave a number that I should call and a reference number: ***, I called again and they changed the story and gave me a case number which was: ***.I called since the 14 August 2014 until the last day which is the 08 October 2015 and only now they tell me the case has been closed. Please help I was to get married but I cannot because I am still married to a stranger. I want you to assist me to get my new I.d with my real surname.
