Waiting for birth certificate for more than 10 mnt

Last year June we went to the DHA Benoni branch to apply for Surname change for my boy.<br> We spend almost the whole day at the offices because the lady who was responsible for issuing forms kept giving us incorrect forms to complete, and when we submit we were informed that the forms are incorrect.<br> <br> We were actually sent back with incorrect forms 4 times until the Office Manager intervened and the lady was removed from the desk were forms were being issued.<br> <br> We finally completed the applications and paid all the monies they requested, we were told it would take about 2 months before the new birth certificate is issued, however to date we still do not have the birth certificate<br> <br> I called the call centre about 4 times I am informed that go back to benoni to ask them to resubmit the forms, when I go to benoni they tell me that the forms are send already and they wont send them again.<br> with the latest scandals of peoples id numbers and birth certificate being hijack at the home affirms offices, I am worried that my sons birth certificate would be duplicated as well as his id number.<br> Can someone please assist in getting someone at the home affairs to get them to issue the birth ce
