unabridged certificate

i app**** for an unabridged birth certificate for my child in January 2015. I was told it takes 8weeks for this to be done and i would receive an sms to collect. In april i had still not heard or received an sms from home affairs. Went to their offices was told to fill in another form, when i questioned why i need to refill in the same form i was told that their head office in PTA requesting same,which was a untrue. Later it came to my knowledge that the first application was never submitted due to an *********** worker. To date i have not received or heard aything since. have been calling PTA headoffice and each time i am told that they are busy with the application. NB the turnaround time being way overdue. FRUSTRATED!!!!!!
