System says I'm deceased in home affairs

In 2017 find that I'm deceased in home affairs at Germiston,but I'm alive how can be possible.so the officers told me to go at police station to affidavits with two witnesses that will confirm that I never pass away(deceased) ,then I submit the affidavits in Germiston on 2017/02/02 So they told me everything will be fine I must just come check when I'm free,i keep on going but nothing happening I'm deceased.even last week I went to fnb to open bank account even there shows that I'm deceased so how will get help my number is ********** 666
