No response for application made 4 months back

May you please assist me with my complaint or direct me to the relevant body whom can assist me with my complaint? In November 2014, I made an application with your Centurion office for a confirmation letter stating the reason why my surname was changed whilst I was a minor. This was for school purposes and required by UNISA. <br> I was told that such a letter will be ready in 2 - 3 months. I have been making numerous trips to that office to collect the letter in question. I have been taken from pillar to post, told to come back, told to call the call centre etc. I have already lost a semester to continue with my studies because the letter has not \been processed"as I was told and they said they don't know why and they keep saying they will have to send a request to get it processed. After waiting for months"
