No birth certificate for my 14 month old daughter

I gave birth in Mulbarton Hospital on the 8th of January 2014 and was assured that my baby's birth certificate would be ready by the time I get discharged which was 3 days in hospital.<br> When I received the birth certificate it had an error the father's name was same as our daughter's name. He asked us to complete other forms and he will have this sent to PTA rectify it and this should take not more than 3 months. He said he would phone us once the correct certificate is ready but this did not happen, instead we went to the hospital personally and was presented with the same incorrect certificate and was told that Wesley did not take the certificate to rectify it but took the forms we completed with him. When decided to go to the Home Affairs to find out how far the process was and we were told to complete forms again and make payment, I have the receipt as proof of this payment made. Till today my daughter has no birth certificate and we need to travel outside SA for Easter Holidays.<br>
