missing documents

I have been trying to apply for insertion of the name on the unabridged certificate as well as rectification of my child's name since 2013. I went to the Roodepoort office in December 2015 and submitted the DNA results as well as all the necessary docs that the dep. requested. i have been doing a follow up with the call centre since January. I have 6 reference numbers for all these calls of which i've been told that application is still in process by all of them. Today when i called i was told that they never received the documents from Roodepoort. I do not remember the name of the guy that i dealt with in Roodeport but surely you should be able to trace that information on your system. The person that i spoke to today is Motlalipula Hofmann with case no: ***.<br> I need this matter to be resolved as soon as possible as it has been pending for more than 3 years now.
