Lack of clearly defined processes and procedures

In 2012, I requested The DHA(Department of Home Affairs) in Pongola, KZN to amend my surname from Thwalo to Thabede. I am a resident of Johannesburg and only did the application in Pongola because I was visiting and I was there at that point in time. The application/request is outstanding and unresolved because it requires my fathers Identity Document.<br> <br> I went to a DHA branch to apply for a passport, which I was told I have a outstanding/open request. I called the Pongola Branch to cancel the pending request, as I am unable to obtain a Passport because of a open request and I was told, they are unable to close the request, as everything is processed at H/O. I went to the Harrison branch with an Affidavit and a letter to request for closure and I was told I have to go to the Pongola Branch to close the change of surname request. <br> <br> I would like to cancel the request (personal reasons) but no one seems to know what I need to do to cancel. I was told supervisors have powers to cancel but no one wants to take responsibility because \incase theres **********\" Which I do not see at all.<br> <br> I need a passport in 20 days and based on the lack of information I am unable to obtain one.
