Incompetence and lack of Customer Service here is Scary!!!!

In January 2017, I went to have my son's surname changed, of which this process took 12 months. Finally in January 2018, this changed happened or so I thought, only to find that when I went have the certificate printed the surname was spelt incorrectly, with 1 alphabet left out. Then I was told that I needed to fill other forms for the amendment of the surname, which I did and was advised to come back in a month's time to check the progress. It is now almost 4 months and after going there every month since March, I've been told the same story that it is still not done. So last month I decided to call the Call Centre in Pretoria and was told that they haven't rceived any documents from the Harrison and Plein street office in March and that I must call them and request the tracking number as proof that they did send the documnets as they have advised me when I went there that they did send the documents through. However when I call the harrison office landline number its just rings without being answered the whole date and the office cellphone number for Lesley is always off. This is so disappointing that people out there get paid a salary that they do not deserve as they do not do what they are supposed to and these culprits that capture our information incorrectly continue doing it cause nothing gets done with regards to their incompetence instead they are protected by the system cause when you ask who captured the appliaction no one seems to know, I mean really if that is not poor service and system then I don't know what is. No wonder people end up staying with wrong names and dates of birth because of all this mess at the department of home affairs, isn't accuracy part of their hiring criteria cause this is really frustrating a lot of people and honestly this is not fair to us, if we fill in forms but are not captured correctly and we are still inconvenienced to fill more forms to fix their employer's faults (where are the original forms that we initially filled?) and then the second forms go missing or are stuck in a pile somewhere while our issues are not resolved is beyond me. My son is now registered with a surname that we do not know, what should happen for us to get a better service?
