Impossible to obtain civil documents from DHA

A family member and I app**** for 4 marriage certificates (amongst other documents) from the DHA. Three of these marriage certificates were app**** for more than 2.5 years ago and the other one more than three months ago. I have visited the Vryburg branch where the applications were submitted with no luck. I have sent emails to the director-general about one of these certificates. I have emailed a number of individuals working at DHA, including the minister, and have to date received no feedback. Recently, I finally received two of these marriage certificates, but both contain very serious errors. My grandmother's details have been added to a marriage record of two unknown individuals. I have found the original record of said marriage and the wife's details were literally replaced with that of my grandmother. In other words, this record issued is false as it states that she was married to somebody she was not. In the marriage record of my great grandparents, there are errors such as country of birth and the spelling of names. After more than 2.5 years I have not received marriage certificates for myself or my parents. Can somebody at DHA please help me obtain these documents in the correct form. I have run out of ideas. On top of this, my surname has been changed at DHA without my consent or approval. I found this out when I went to vote.
