Documentation lost and still waiting

DHA made a mistake on my son's birth certificate and printed the wrong date, we went there a month after he was born to rectify this and we were told we had to apply for an amendment. We did this, we were told it would take 3mnths to amend it. 3mnths later, nothing, 6 mnths later, nothing. Next thing we are told we only app**** last month...A WHOLE YEAR later. I am appalled and disgusted by this department. At some point we were told the application was lost, just like that and that was end of story. To this day we are still waiting, birth certificate -less, the worst department.
