Department of HomeAffairs lost my ID book

I trust this message finds you well.
I am writing to urgently request your assistance regarding an issue I experienced on 25 March 2026 when attempting to collect my ID document at the Department of Home Affairs, located at 77 Harrison Street, Johannesburg.
Upon arrival, I waited in line. When my details were checked on the system, it confirmed that my ID was available for collection. My fingerprints were then taken as part of the verification process while officials went to retrieve the document. However, after several minutes, I was informed that my ID could not be located.
When I calmly requested clarification and guidance on the next steps, I was advised—without a clear explanation—that I would need to either reapply or send an email to the Minister directly. I then attempted to seek further assistance from both the attending official and the manager; however, I was met with a lack of clear communication and professionalism.
I was subsequently informed that I would need to:
Visit a police station to complete an affidavit Return to Home Affairs to have my fingerprints removed from the system Reapply and pay for a new ID document
My concern is that the Department has already recorded my fingerprints as though the ID was successfully collected, yet I did not receive the document.
I would appreciate clarity on the following:
What is the official process when an ID document is lost within the Department? Should I be required to bear the cost and inconvenience of reapplying under these circumstances? What steps will be taken to resolve this matter efficiently and fairly?
I respectfully request your guidance and intervention in resolving this matter, as I rely on this document and would like to ensure that the correct process is followed.
