Dearth certificate

Iam Phindiswa Goba i would like you to help me with my dearth certificate of my late mother who passed away on 2005 at Grey Hospital in King williams Town bt the problem is that when i went to home affairs in capetown they say my mother is stl alive on the system i juat want to find out how n why cz he passed away years Ago so how come what happened to the body of my mother
