Bad services and incorrect processing

I had app**** for a change of forename on my ID. I am disappointed to had called and heard that my name was successfully changed then reversed. I am starting to suspect this is an act of xenophobia but I am hoping it is someone's mistake that needs to be rectified immediately. Who do I speak to and how can this be corrected?
