Bad and unreliable service from The Department Of Home Affairs

On 19 April 2018 I requested a copy of the Notification Of Death (BI-1663) certificate/document at the Bellville Home Affairs department. On several occasion I tried to request feedback regarding the status of my request but was disappointed by unsuccessful results via the Contact Centre ********** 190. I was either told that the system is down or they don’t understand my request for a copy of the Notification Of Death certificate/document.

My family need a copy of the certificate/document in order to settle unresolved matter pertaining to my deceased grandmother.

Is it possible that anyone can help me by providing me with a copy of the Notification Of Death (BI-1663) certificate/document?
