Traces of glass under my stiches

I am so appalled by the level of hygiene at Lesedi Hospital, i had an accident last year in September. I had an open wound on my leg and when i was rushed to Lesedi Hosp, the DR on duty confirmed the wound was deep and needs to be stitched up. I was under the impression that my wound will be cleaned up before the stitching. i went though check up's and all and eventually removed the stitches, i was horrified end of October when my wound started swelling and when i was cleaning the wound, i discovered that there were traces of glass underneath the stitches poking me. My wound was becoming septic, so i had to remove the glasses myself. I called Lesedi hosp the next day to explain what had occurred, some Dr said that in order for them to examine me again i had to pay and that was about +-R800 cash. My med aid had exhausted and i could not go. I still feel things under my stitches till today, is it not the responsibility of the hospital to take care of their patients? This was a horrible error done by the Hospital and not me. What should i do to remove the glasses still under my stitches?????
