TB Meningitis

I am a Brother to a Patient Sibongile Margaret Mdubeki she was diagnosed at Ritchflei Hospital (Mhlangeni) last year of the TB Meningitis. From then she's been getting in and out of Hospital without improvements instead it getting worse. Now as her family we do not understand if she take medication or not when she's in hospital, because we have just noticed that her medication card was last ticked on the 19th of April 2015 and she was discharged by the hospital on the 02nd of May 2015, how come sisters and nurses did not notice that she was not taking the medication, as Sbongile has already got confusion. We've been trying to get answers from the Dr and staff nurses with no luck. At some months ago her file was lost inside the hospital, how did that happen?
