Missing Patient Report Form (PRF)

My name is Mohammed Olanrewaju, on the 12th November 2014 I was involved in a Motor vehicle accident around R566 at night. I was unconscious when the paramedics transported me from the accident scene to Brits hospital. My complain is that the Patient Report Form that was submitted by the paramedics went missing which make it difficult for me to open up an Accident Case so that I can claim from Road Accident Funds. I have made several visits to the hospital in search of that form but I was sent from one pillar to post. No one wants to account of what happened to my Patient Report Form.
