Complant against Thembalethu Clinic in George Western Cape

I want to log a complaint againts Sindile April who works at Thembalethu clinic in George western Cape. He is very careless, disorganised, ***********. Friday, my Mom went there to report that she forgot my child's clinic book in Carltonville and she thinks his date is today because hes turning 2 today. Sindile guy told her to come today with birth certificate and she must come very early because he must look for a file. So today I went there with my child very early 06:45 to be exact, I queued outside because clinic was still closed. They opened at 07:30 and we went in to 8. I gave him the birth certificate, explained again that my mom came on friday to report that she forgot clinic book in GP, she thinks the appointment is today and you said she must come with a certificate. He asked if the child uses Thembalethu clinic, I said yes and he only has a file there. He said there is no problem then they will look for a file and he put the certificate together with other kids clinic book. I stayed there the while day waiting for my child's name to be called but nothing. They called everyone, even those who came late than me. I went to a Nurse and asked if she has my son's file, said no, i went to another one, she also said no and she advise me to go to reception and ask them before they close and the time then was 15: 47. I went there, told them the story again. They said the birth certificate has been there with them the while day, They asked Sindile about this certificate he did noy know about it and couldn'd tell them anything. They check the appointment date on the system for me, told me its not today its the 26th. After spending the whole day at the clinic, i went home without being helped, I was told to come back on the 26th which is something he could have checked for me in the morning or after handing all todays files to the nurses. I would like to get a response to update me if you have taken action because its not the 1st time hes is giving pathetic service. I will then follow up.
