No response from Erna Steyn

I have been trying to get hold of Erna Steyn for a month now and I can't get hold of her...She needs to issue me out with my form 17.W form and she is ignoring my Emails and my calls...I am really fed up with this issue...I need assistance urgent with regard to this Matter.
